After filling out your sponsorship form with payment, or choose to donate the sponsorship custom amount on our square checkout site, please email the form to admin@achhs.org. If you would like to send conference attendees, please go to square registration, scroll down to the last ticketing option titled Sponsor Complimentary Registration to register up to two sponsored guests.
